Skilled Nursing Unit
Our Skilled Nursing Unit is a 64-bed nursing and rehabilitation center located at Garnet Health Medical Center - Catskills, Harris Campus. We pride ourselves on providing the highest quality of skilled nursing care for individuals requiring both long-term care and short-term rehabilitation.
Whether you’re here for a short-term rehabilitation stay after a traumatic injury or long-term for eldercare or assisted living services, we are committed to promoting the highest level of independence and the best skilled nursing care in a home-like setting. Our dedicated team will work with you to develop an individualized plan to meet your or your loved one’s specific needs and goals.
Skilled Nursing Care Planning
Each resident of our Skilled Nursing Unit will benefit from a personalized care plan designed to address their specific needs and goals. The care plan is developed by an interdisciplinary team composed of our healthcare professionals, in addition to the resident and family or representative.
Within 14 days of admission, you will be invited to attend a care planning meeting. Please inform our staff of any special needs, habits or preferences prior to this meeting. Information can be added to the care plan at any time.
Long Term & Short Term Nursing Care
The assisted living, rehabilitation, and skilled nursing home services provided at our Skilled Nursing Facility at Garnet Health Medical Center – Catskills allow for complete access to the full range of medical services offered at the hospital – 24-hours/day, including:
Our Skilled Nursing Unit Professional Team also is compromised of: Licensed Nursing Home Administrator, Registered Nurses, Licensed Practical Nurses, Certified Nursing Assistants, Certified Social Worker, Certified Activities Director, ADA Registered Dietitians, Licensed Physical Therapists, Licensed Occupational Therapists, and Licensed Speech/Language Pathologists.
Our nurses are trained professionals performing services that are needed for long-term care or short-term situations due to an injury or illness. Our nursing staff will attend to wound care needs related to post-operative, pressure and stasis ulcers.
Nurses are also responsible for dispensing and monitoring of oral, intravenous and subcutaneous medications. Respiratory care may also be performed; this can include oxygen, C-Pap, Bi-Pap and nebulizer treatments. Palliative and end-of-life care is offered as needed.

We are dedicated to promoting the highest level of independence and the best medical care in a home-like setting. We offer both private and semi-private rooms with breathtaking views of the Catskill Mountains.
Along with our medical services provided, residents will also enjoy as part of their care an array of daily activities and personal-care services provided by our Skilled Nursing Unit, such as:

Residents have spacious activity rooms, lounges, sunrooms and internet access. During the warmer months, residents are able to enjoy the beautiful Woodland Wildings Garden, located on the grounds for both residents and their family members.
Emergency Situations at the Skilled Nursing Unit
The Skilled Nursing Unit also offers the distinct advantage over other nursing homes or senior care living facilities, as we are on the main campus of Garnet Health Medical Center – Catskills. In the event of a medical crisis, qualified medical experts are just a few steps away. In addition, a great emphasis has been placed on rehabilitation and successfully transitioning residents back in to the community. The therapy team provides resident specific programming right in the unit.
In the event of an emergency, your personal physician will be contacted. Depending on the nature of the emergency, your doctor may either personally evaluate you or request that you be sent to the emergency room for evaluation.
The facility will notify one contact person in the event of a medical emergency or change in your condition. In the event that the primary contact person is not available, we will contact an alternate. The contacted individual is responsible for sharing information with other family members and friends.
In the event of a life-threatening emergency, such as a cardiac or respiratory arrest, CPR will be administered under the direction of a responding physician, unless a do-not-resuscitate (DNR) order is present.
Pandemic Emergency Plan
To protect the well-being of residents, staff, and visitors, the following all-hazards Comprehensive Emergency Management Plan (CEMP) has been developed and includes considerations necessary to satisfy the requirements for a Pandemic Emergency Plan (PEP).
The purpose of this plan is to describe the facility’s approach to mitigating the effects of, preparing for, responding to, and recovering from natural disasters, man-made incidents, and/or facility emergencies.
