We stay in touch with patients through MyChart, our secure online portal. It is highly recommended that you sign up for this quick and easy access to all your health records. With MyChart, you can:

• Request prescription refills

If you have questions about your upcoming appointment, call us at 845-333-7575. We also have an on-call doctor available after hours through this number.

Before your visit, please:

• Let your insurance company know we’re your primary care provider.
• Bring your medical records or ask your doctor to send them to fax_ghvhs@verisma.com.
• Bring a list of your medications, including strength and daily doses.
• Bring your insurance card and photo ID.
• If you're accompanying a child, please bring guardianship documents, if needed.
• Arrive 15 minutes early to check in.
• Be ready to pay any copays for your visit.
