Important health coverage tax documents (1095-C Form)
Form 1095-C for the 2025 tax year is available upon request. The BEE Award, celebrates any member of our hospital family who demonstrates excellence in their interactions with patients, visitors, colleagues and the community. Individuals can be nominated for this award. To nominate a staff member, email the name of the individual and a description detailing why you are nominating them to: BEEaward@garnethealth.org
