About this event

Program consists of 16 weekly sessions, followed by 6-8 more sessions conducted monthly. The September 16 meeting is an informational session, no commitment required. The course begins on September 23. All classes are led by a certified life coaches: Delilah Socci, MSN, Garnet Health Community Health Manager. This is an in-person class. English and Spanish.


Through year-long sessions and with the support of a group, lifestyle coaches trained to use a CDC-approved curriculum help you stay motivated to:

  • Make small changes to the way you eat without giving up the foods you love
  • Increase your physical activity levels to 30 minutes a few days a week
  • Manage your stress

The program follows a research-based curriculum that starts with weekly group meetings for the first six months, followed by upkeep sessions to keep you on track to meet your goals.

Qualified participants will work together to achieve their goals, under the guidance of our certified lifestyle coaches.

Our Diabetes Prevention Program is offered to eligible individuals with prediabetes. To be eligible for our program, participants must;

  • Be at least 18 years old
  • Be overweight (body mass index ≥25; ≥23 if Asian). Calculate Your BMI on the CDC Website
  • Have no previous diagnosis of type 1 or type 2 diabetes
  • Must have any of the below risk factors:
    • Received a high-risk result (score of 5 of higher) on the CDC's Prediabetes Risk Test
    • Been previously diagnosed with gestational diabetes
    • A blood test result in the prediabetes range within the past year:
      • Hemoglobin A1C: 5.7%–6.4%
      • Fasting plasma glucose: 100–125 mg/dL
      • Two-hour plasma glucose (after a 75 gm glucose load): 140–199 mg/dL
