Garnet Health Medical Center Achieves Recognition

MIDDLETOWN, NY (Oct. 6, 2025) — We are thrilled to announce that our hospital has been recognized as an Age-Friendly Health System Participant by the Institute for Healthcare Improvement (IHI). This significant achievement underscores our commitment to providing safe, high-quality health care tailored to the individual needs of older adults.

A Commitment to Age-Friendly Excellence

In February 2025, our hospital joined the IHI Age-Friendly Health Systems Action Community, committing to the IHI's standards for Age-Friendly care. This initiative focuses on the "4Ms" Framework: What Matters, Medication, Mentation and Mobility. Our dedicated teams on pilot units 3 South, 4 North, and 5 North have been instrumental in this journey. They have successfully implemented creative strategies to enhance the patient experience, including: distributing engaging activities for older adults, safe mobility practices, focusing on patient-specific goals and preferences, and screening for delirium and malnutrition. These initiatives demonstrate our commitment to ensuring that every older adult receives personalized, compassionate, and effective care.

This achievement is a testament to the power of teamwork. Numerous departments collaborated to make this possible, including: Quality, Guest Services, Nursing, Information Technology, Patient Experience, Pharmacy, Rehabilitation Services, Volunteer Services, Food and Nutrition, and Nursing Professional Development. Their combined expertise and dedication have been crucial in integrating Age-Friendly practices throughout our hospital.
